Why refined starches beat out sugar as the worst offender

When I explain this to clients, they’re always surprised. They expect candy bars or soda to be the worst. But research tested 62 common foods against pure glucose and found vegetables produced the largest glucose rises at 70%, followed by breakfast cereals at 65%, then refined grain products at 60%. Table sugar came in lower.

The reason is speed of digestion. Refined starches have been stripped of fiber and protein. They break down into glucose almost immediately.

Your body absorbs them fast, flooding your bloodstream with glucose before insulin can catch up.

One of my clients switched from white toast at breakfast to sourdough with almond butter. Her continuous glucose monitor showed her morning spike dropped from 180 mg/dL to 125 mg/dL. Same meal timing, same insulin dose. The only change was the type of carbohydrate.

These foods have glycemic index values above 70, putting them in the high category. For comparison, pure glucose is 100.

Research using continuous glucose monitoring in youth with type 1 diabetes found high-GI meals produced significantly higher mean blood glucose and more time spent above 180 mg/dL compared to macronutrient-matched low-GI meals. The carbohydrate content was identical. Only the speed of digestion changed.

What are the 5 worst foods for blood sugar?

Based on glycemic response and how commonly people eat them, these five foods cause the most problems:

1. White bread and white flour products

White bread has a glycemic index of 75. Two slices can spike blood sugar as much as drinking a can of soda. The refining process removes the bran and germ, leaving only the starchy endosperm that digests rapidly.

2. Corn flakes and puffed cereals

Corn flakes score 81 on the glycemic index. The processing puffs or flakes the grain, increasing surface area and making it digest faster. I had a client who ate corn flakes every morning and couldn’t figure out why her fasting glucose was high by 10 AM. We switched her to old-fashioned oatmeal with walnuts. Her glucose stayed under 140 mg/dL all morning.

3. Instant mashed potatoes

Instant varieties score around 87. Regular russet potatoes are already high at 78, but the industrial processing makes instant versions digest even faster. The potato has been cooked, dehydrated, and turned into flakes that rehydrate in seconds.

4. White rice, particularly instant

White rice ranges from 64 to 93 depending on variety and cooking method. Instant white rice is at the top end. Brown rice scores 50 to 55. Replacing white rice with brown, quinoa, or barley drops the glucose response significantly.

5. Sugar-sweetened drinks

Soda and fruit juice deliver concentrated glucose and fructose with no fiber, protein, or fat to slow absorption. A 12-ounce can of soda contains 39 grams of sugar that hits your bloodstream in minutes. Liquid carbohydrates bypass chewing and digestion, making them uniquely fast.

What 7 fruits should diabetics avoid?

Most whole fruits are fine for blood sugar control because they contain fiber that slows glucose absorption. The fruits to limit are those with very high sugar content or those consumed in processed forms.

Fruits that spike blood sugar faster:

  • Watermelon (glycemic index 76)
  • Pineapple (glycemic index 66)
  • Overripe bananas (glycemic index increases as they ripen)
  • Dried fruits like raisins and dates (concentrated sugar, no water volume)

In my experience working with diabetic clients, whole fruits like berries, apples, pears, and citrus cause minimal blood sugar issues when eaten in normal portions. The fiber content and slower digestion keep the glucose rise manageable.

When you eat a whole potato, your body has to break down the intact cell walls and starch granules. When you eat instant mashed potatoes, that work has already been done. The starches are exposed and ready to convert to glucose immediately.

What should a diabetic never eat?

There’s no single food that is absolutely forbidden. But some foods make blood sugar control so difficult that avoiding them is the practical choice.

Foods to eliminate or severely limit:

  • Sugar-sweetened beverages (soda, sweet tea, energy drinks, fruit juice)
  • White bread, white rice, and products made from refined white flour
  • Breakfast cereals with added sugar or made from refined grains
  • Pastries, donuts, and baked goods made with white flour and sugar
  • Candy and sweets without fat or protein to slow absorption
  • Fried foods coated in refined flour breading

These foods combine high glycemic index with large portion sizes, making it nearly impossible to dose insulin correctly or maintain stable blood sugar.

I remember when one of my clients kept having unexplained afternoon crashes. We went through her food diary and found she was eating two slices of white toast with jam mid-morning. Her glucose would spike to 200 mg/dL, her body would release extra insulin, and by 3 PM she would drop to 65 mg/dL.

Use this decision framework every time you choose a carbohydrate-rich food:

Choose intact grains over flour products. Steel-cut oats beat rolled oats. Whole wheat berries beat whole wheat bread. The more processing, the faster the glucose spike.

Look for visible fiber and texture. If you can see seeds, bran, or whole grains, the food will digest slower. Smooth, soft, uniform texture usually means high processing.

Pick the version that takes longer to chew. An apple takes time to chew and fills your stomach. Apple juice goes down in seconds. The act of chewing and stomach volume both slow digestion.

Read the ingredient list, not just the nutrition label. Two breads can have identical carbohydrate counts but wildly different ingredients. One made from whole wheat, water, and salt will affect blood sugar differently than one made from enriched wheat flour, high-fructose corn syrup, and dough conditioners.

Pair carbohydrates with protein or fat at every meal. This is the single most effective strategy. Even if you eat a higher-GI food, adding protein or fat reduces the glucose spike significantly.
